Choose your service model

What will your agency provide?

North Carolina’s home care framework includes nursing, in-home aide, companion, sitter, and respite services. Your intended services shape the staffing, policies, and documentation your agency needs.

Non-medical home care

Support with daily living.

For agencies planning in-home aide and personal care services, companion services, sitter services, or respite care.

We help clarify your intended scope and prepare licensing materials and policies around the services you select.

Skilled nursing services

A foundation for nursing care.

For home care agencies planning to provide nursing services, alone or alongside non-medical services.

We review the licensing pathway and the clinical leadership, personnel, policy, and documentation requirements relevant to your planned scope.

Ask about skilled nursing

Planning Medicare-certified home health? That involves a distinct certification pathway. Offering nursing through a home care agency does not automatically establish Medicare certification. Explore home health support

Prepare for your next step

What should you have in place?

You do not need to have everything completed before contacting us. These are the areas we help clients review when defining the appropriate support.

Business entity

Whether your business has been formed and what administrative setup remains. Entity documents and related filing services can be discussed when needed.

Planned services

Identify whether you intend to provide non-medical care, nursing, or both, so application materials and policies reflect your actual scope.

Agency leadership

Identify your proposed agency director and review the qualifications applicable to your intended services.

Staffing & supervision

Review the personnel and oversight your service model requires, including clinical leadership where applicable.

Policies & documents

Determine which policies, supporting records, and application documents are ready and which still need preparation.

Agency leadership qualifications

Who can serve as your agency director?

The applicable standard depends on your services. The companion-only pathway should not be used for an agency providing hands-on in-home aide or nursing services.

In-home aide and nursing agencies • 10A NCAC 13J .1001(b)

The director needs at least one of these:

  • Health care practitioner status under G.S. 90-640(a).
  • Two years of supervisory or management experience in home care or a provider licensed under G.S. 131E or G.S. 122C.
  • A bachelor’s degree in health, business, or public administration science, plus one year of supervisory or management experience in home care or another licensed health care program.
Companion, sitter, and respite-only agencies • 10A NCAC 13J .1503

A high school diploma or GED is required, plus either health care practitioner status under G.S. 90-640(a) or one year of experience in home care, companion, sitter, respite, or a provider licensed under G.S. 131E or G.S. 122C.

Agency director and nursing supervision are distinct roles

Meeting director qualifications does not establish nursing qualifications. For in-home aide services, DHSR requires an RN available for supervision and consultation during service hours. Review clinical staffing separately.
